Scheduling for a BUS or VAN Physical?

Be aware if you are DIABETIC and on any prescription medication you will need to have waiver DL-122 A completed by your Primary Care Provider prior to your appointment

Are you diabetic and currently prescribed INSULIN? You will need form MCSA 5870: Click here for the form

Do you have MONOCULAR VISION? Click here for the form that will need completed MCSA 5871

Do you have Sleep Apnea and wear CPAP? You will be asked to provide proof of daily and hourly use of CPAP compliance.

Medical Verification Form: You may be asked to have this complete based on your current list of medications and would need completed by your Primary Care Provider: MCSA 5895

Are you diabetic and currently taking any prescription medication for your DIABETES?

You will need your Primary Care Provider complete waiver DL-122 A. Follow this link to form that needs to completed online. Form DL-122A Diabetic Waiver
